http://brignonimd.com/placing-silicone-breast-implants-vs-saline-breast-implants/ Florida cosmetic surgeon, obstetrician and gynecologist Dr. Anthony Brignoni explains the difference in placing silicone breast implants vs. saline breast implants.

The biggest variance is the method by which the implants are placed: both are placed in a pocket created behind the breast muscle.

But the saline breast implant is able to go in through a smaller incision because the saline implant is not filled until it is inserted. When it's in place a saline solution fills it like a water balloon.

The silicone implant is placed inside the breast but it is already filled with silicone gel so it requires a larger incision. However, there is a new Kennel funnel that allows the breast implant to be slid in through a smaller incision. It costs a little bit more but it will be able to be placed.
