The woman was in the process of getting the horse used to being mounted and dismounted when her shirt got caught on the saddle horn. This caused her to inadvertently knee the horse in the stomach while trying to get back up. In response to the knee, the horse jumped and galloped forward, throwing the woman to the ground.
