29 deaths at Launceston General Hospital to be investigated by Tasmanian coroner after senior staff misconduct

An independent panel has found 19 deaths at the Launceston General Hospital should have been referred to the coroner but were not. The panel’s report says a former senior staff member consistently certified deaths in ways he was not legally allowed to and the report has been referred to Tasmania Police and the Integrity Commission.
