Queensland authorities failed to heed warnings that two toddlers were at risk before they died in their mother's hot car in 2019. Yesterday Kerri-Ann Conley pleaded guilty to manslaughter for the deaths of her daughters, two-year-old Darcey-Helen and one-year-old Chloe-Ann.
