A Probe into the AirAsia plane crash which killed 162 people has found the disaster was caused by “crew error”. Indonesian investigators said today a chronically faulty component and the crew's attempt to fix the problem were among the factors that contributed to the crash of an AirAsia passenger jet last year that killed all 162 on board.
