A Philadelphia bus driver is facing disciplinary action because she was not buckled in when she fell out of her seat and nearly hit a freight train. A video shows the driver slipping to the floor while she grabs for the wheel. The bus jumped a median and veered toward the train before coming to a halt on the tracks.
