Don Berwick, senior fellow at the Institute for Healthcare Improvement, envisions a health care system with comprehensive electronic records but warns, "Don't confuse the automation of a record with a change in the delivery of health care. We need a new health care system. We need one that is organized around prevention, spirituality, home- based care."
